DOCUMENTS FOR ALL CLASSES
-Registration Certificate from NADRA
-School leaving certificate (Original)
-Result card issued by previous institution
-Four pasport size blue background colored photographs
ADDITIONAL DOCUMENTS FOR BOARD CLASSES
-Provisional character certificate issued by previous institution.
-Detailed mark sheet issued by the board -Migration certificate (if migration from the other board)
Affidavit /authorized form etc.
If, at any stage, it is found that any document submitted by a student is false or tempered with the admission will be cancelled.
MEDICAL FITNESS CERTIFICATE
Only medically fit students can apply for admission. Those suffering from any communicable / infectous diseases shall not be granted admission.
Health cerficate must be from Al-Qasim Hospital Murree Road Bhara Kahu, Islamabad.
